| Cancellation
Policy |
It is our policy that all
cancelled appointments require a minimum 24 hours notice. We reserve this time
especially for you; cancelling on short notice disrupts the scheduling and
treatment of other patients. Appointments cancelled without 24 hours notice are
subject to a $35.00 fee. We understand that emergencies do occur; we treat this
policy on a case-by-case basis.

| Patients With
Insurance Coverage |
Dr. Gordon does
participate with several insurance companies. For those patients that have
dental insurance, we will be glad to assist you with your claim. We do,
however, require co-payment at each visit for the treatment provided and
payment in full by the time the treatment plan is
completed.
We submit most claims
electronically for expedience, however you are responsible for any portion of
the fee not covered by the insurance.
For major dental treatment such as crowns, bridges, implants,
dentures and orthodontics, a payment plan schedule may be arranged, however all
payments must be made by the completion of treatment.

| Patients Without
Insurance Coverage |
If you do not have dental
insurance benefits, we can work out a manageable payment schedule with you. We
will do our best to make your dental treatment affordable and within your
budget.
